Blue Ridge CTC is adopting a new policy for Release of Information. Releases completed in Bridge are no longer available. You must complete this form in person at the main campus. If you are unable to come in person, please complete both pages and have it notarized.

Please note: You MUST sign this form in front of a member from the Registrar’s Office. Do not sign until you are in our office and working with a member of the Registrar’s Office. The only exception to this is if you are getting it notarized.
