Job Location (City, State): Hagerstown
Job Description: This employee will transport patients to and from their homes to the health center for appointments for primary care, dental care and behavioral health services. She/He will assist with scheduling transport and placing reminder calls; ensuring vehicle maintenance is current and ongoing; maintaining accurate daily reports; and assisting the Facilities Assistant as requested. Requirements: the candidate must be 21 years of age with at least 5 years of driving experience; have a current and valid driver’s license; have a clean driving record; have graduated from high school with commercial/business course or the equivalent; have knowledge of area roads and highways; have the ability to recognize vehicle maintenance needs and perform minor vehicle repairs; have experience working with diverse populations; have excellent customer service and interpersonal skills. Job Title: RN Case Manager
Job Location (City, State): Hagerstown
Job Description: The Case Manager is responsible for managing chronic illness patients in a patient-centered model to promote effective education, self-management support, and timely healthcare delivery to achieve optimal quality outcomes. The employee will collaborate with the care team and coordinate treatment while prioritizing patients according to complexity and needed follow-up. The person will maintain databases, provide timely documentation, and schedule patients appropriately. He/She will identify and utilize community resources as necessary and according to plan. The role includes coverage of the triage phone, gathering statistics for grants, and maintenance of AED’s. The candidate must have a current Maryland RN license; prior family practice case management experience is preferred.
