Admissions & Credits Petition – special approval to do something out of the ordinary

Academic & Personal Information Change – Used to change program, concentration, or advisor AND used to change Name, Social Security Number, or Permanent Address

Academic Forgiveness – student who is returning to college with a grade point deficiency

Course Change Form - requests permission to register over the maximum 19 credit hours in a specific semester (14 hours in a summer term), or change from one section of a class to another (same class), or change from credit to pass/fail or audit.

Graduation Application/Change Form – apply for graduation or change a student’s graduation date or catalog term

Transcript Request – request an Official Transcript

Transfer Approval – students who wish to take classes elsewhere and transfer back

Verification of Enrollment – to prove enrollment (typically for insurance)

Withdraw – be removed from classes in a specific semester (partial and complete)
