Current Students: Graduation Application/Change Form
When to use this form:
- For students planning to apply for graduation
- For students who need to change their graduation date, if they are not going to graduate in the semester that they indicated on their application
Who needs to sign?
- Student
- Advisor
How to complete:
- The student picks up the form and fills out the information requested
- Be sure to complete any sub forms that are not displayed on the Degree Evaluation
- The student and advisor then sign and return the graduation form to Enrollment Management
Other Information:
- A sub form must also be used for course subs or waivers
- Be sure to check the catalog year for students, this is important because that is the information that the Office of the Registrar pulls for degree evaluations
