Current Students: Graduation Application/Change Form

When to use this form:

  • For students planning to apply for graduation
  • For students who need to change their graduation date, if they are not going to graduate in the semester that they indicated on their application

Who needs to sign?

  • Student
  • Advisor

How to complete:

  • The student picks up the form and fills out the information requested
  • Be sure to complete any sub forms that are not displayed on the Degree Evaluation
  • The student and advisor then sign and return the graduation form to Enrollment Management

Other Information:

  • A sub form must also be used for course subs or waivers
  • Be sure to check the catalog year for students, this is important because that is the information that the Office of the Registrar pulls for degree evaluations
