Current Students: Academic & Personal Information Change Form
When to use this form:
- Change a major
- Change a concentration
- Change a advisor
- Changes a students name (with documentation such as a marriage certificate or divorce papers)
- Changes a students social security number (with proper documentation)
- Changes a student’s permanent address.
Who needs to sign?
- Student
- New Advisor (if changing or adding major)
- Old Advisor (if changing or adding major)
How to complete:
- the student picks up the form and fills out the information requested (completing only the sections that are necessary for them - i.e., they do not need to fill out the social security number section if they are changing their address)
- If the student is changing advisor or major they will need to obtain the advisors signature
- The form is then returned to Enrollment Management and the change is made in banner
Other Information:
- When signing the form, be sure the student has filled out all sections according to what they are changing and has signed the form
- Students are NOT able to complete this information in Bridge
