Current Students: Withdraw

When to use this form:

  • Is used for students to withdraw from some or all of their classes.

Who needs to sign?

  • Instructor
  • Advisor
  • Student Affairs
  • Financial Aid
  • Student

How to complete:

  • The student picks up the form and fills out the class information requested.
  • The student takes to the instructor of the class(s) they want to drop and has them sign it.
  • The student takes to their academic advisor for them to sign (advisor will inform them of any problems this may cause with graduation)/
  • The student then obtains other signatures required, and they are then informed of any problems they may encounter by withdrawing.
  • The student returns the form to Enrollment Management with appropriate signatures.

Other Information:

  • Withdrawing from courses may interfere with a students ability to graduate on time.
  • Dropping below 12 credit hours in a give semester may make a student ineligible for financial aid.
  • There are no refunds for partial withdraws, only complete withdraws.
