When to use this form:

• For students who want to be classified as an instate resident.

Who needs to sign?

• Student
• Parent or Guardian (if student is a dependent)

How to complete:

• The student picks up the form and fills out the information requested on the front.
• The student must gather the appropriate documentation for the application as outlined on the application.
• The student then signs and returns the form to the Director of Enrollment Management.

Other Information:

• There are very strict guidelines for this form; encourage the student to completely read the guidelines on the third page.
• This form requires extensive amounts of documentation to prove instate residency.
