Current Students: Graduation Date Change

When to use this form:

  • For students who need to change their graduation date, if they are not going to graduate in the semester that they indicated on their application.

Who needs to sign?

  • Student
  • Advisor

How to complete:

  • The student picks up the form and fills out the information requested on the front.
  • The student and the advisor sign and return the form to Enrollment Management.

Other Information:

  • If a student does not complete this form and does not graduate, their status will be moved to the next semester.
  • A student only needs to pay the Graduation Fee once for each degree earned.
  • Pay careful attention when you are signing graduation evaluations - this will eliminate the need for this form in many instances.
