Current Students: Course Substitution or Waiver

When to use this form:

• Substitute or waive a requirement for one student for graduation.

Who needs to sign?

• Student
• Advisor
• Program Coordinator

How to complete:

• Typically the advisor completes this form along with the student.
• They both agree on the courses to be substituted or waived and complete the course information along with the justification for the substitution.
• The student and the advisor sign and it is returned to Enrollment Management.

Other Information:

• Be sure to confer with the student in the process.
• Complete this form when the agreement is made. Do not wait until the student is ready for graduation.
• This substitution can also be made to classes that are in progress.
